We all experience change in our lives. Some changes we can adapt to, and others might require us to try a new direction. This is where the NEK Council on Aging can help.
The NEK Council on Aging has experienced staff who can conduct a comprehensive assessment to determine how best to meet your in-home needs. They also can identify programs that empower individuals and families to make informed decisions, such as Vermont’s Choices for Care (long-term care Medicaid) and the Veterans Directed Care Program, a veteran-directed home and community-based program which offers support to eligible participants.

Our Options Counselors are here to help you to live independently by discovering the opportunities available to you. These may include assistance with applications for food (3SquaresVT) or fuel, providing direction for help with housing, or assisting with the application for Choices for Care.
Case Managers help coordinate and monitor supports for those who need ongoing assistance. They will work with you to problem-solve and find available resources to address your needs. There are several programs offered in the State of Vermont for which you can qualify to remain living independently. The Council is also a key partner with other health-care and human-services organizations (NVRH, Northern County Health Care, North Country Hospital, Caledonia Home Health and Hospice, etc.) who may support your care at home.

After a stay at the hospital, rehabilitation, or nursing home, transitioning home can be confusing and overwhelming. Our staff can help you navigate this difficult time. Before discharge, Council staff will meet with you to assess your needs, discuss your options, set up meetings and/or service (including other providers), and assist with developing a plan for your care. Our staff has the knowledge, capability, and desire to help you continue to improve and thrive upon your return home. We are here to support you during the entire transition to home and your continued care.
