We all want to live independently and safely in our own homes despite the challenges that can come with changing health, mobility, and/or chronic medical conditions.

The NEK Council on Aging has experienced case managers who can conduct a comprehensive assessment to determine how best to meet your in-home needs. They also can identify programs that empower individuals and families to make informed decisions, such as Vermont’s Choices for Care (long-term care Medicaid) and the Veterans Independence Program (V.I.P.), a veteran-directed home and community-based program which offers support to eligible participants.
